Parents Completing Annual Validations

Parents,

It's that time of year again!

Every Cadet requires the updated annual validations for their files. This will cover any changes to address and phone number.  When completing the form, just prior to your signature is a "yes" or "no" box. This is a very important box and must be completed correctly.

It advises us if there has been any change to your child's medical profile. If it is not completed we can not validate your child's profile. If the medical has changed we need an updated Medical form to be submitted to Winnipeg. If it has not changed please check the "no" box.
